Diabetes is one of Ghana's fastest-growing health challenges. Managing diabetes well at home is one of the most powerful things you can do for long-term health. This guide explains what proper home-based diabetes management looks like — and where CareEdge fits in.

What diabetes requires at home daily

Diabetes is not a condition you manage once a week at a clinic. It requires daily attention — blood sugar monitoring, medication adherence, diet management, foot care, and wound awareness. When any of these elements slip, the consequences can be severe.

Blood sugar monitoring

Blood glucose should be checked regularly — ideally before meals and at bedtime. The target range for most diabetic patients is 4 to 7 mmol/L fasting, and below 8.5 mmol/L after meals. Readings outside this range need to be acted upon — not ignored until the next clinic visit. A CareEdge nurse checks blood glucose on every visit and flags abnormal readings to the doctor immediately.

Foot care — critical for diabetic patients

Diabetic foot care is not optional. Diabetic patients often have reduced sensation in their feet due to nerve damage, meaning they may not feel a wound forming. Daily foot inspection, keeping feet clean and moisturised, and wearing appropriate footwear are critical. Our nurses conduct a formal foot assessment on every visit for diabetic patients.

Diabetic wounds are emergencies. A wound that seems minor today can become infected and life-threatening within days. Never wait to see if it improves on its own.

Diet management in the Ghanaian context

Diet management for diabetes requires practical, culturally appropriate guidance — not generic Western advice. Our clinical team advises on how to adapt traditional Ghanaian meals to manage blood sugar effectively, without abandoning the foods your family has always eaten.
